This form is for skaters who want to participate in a Skating Clinic with Tui Lyon, an Angel City Derby skater since 2016.

Clinic Levels:

$55 - Full Day Clinic

• Saturday 5/24 10 AM to 3 PM

• Admission to Spring Fling Smash-up at 6 PM

• Includes lunch and 1 hour off-skates debrief

$75 - Full Day Clinic and Smash-Up

• Saturday 5/24 10 AM to 3 PM,

• Registration for Spring Fling Smash-up at 6 PM

• Includes lunch, 1 hour off-skates debrief, and personalized game shirt

Location: Medford Armory 1701 S Pacific Hwy Medford, OR 97501

Participation Requirements: All attendees should have active WFTDA or USARS insurance, open to WFTDA Gender Expansive 18+, Contact experienced Skaters.

What to Bring: Notepad, pens, pencils, skates, safety gear (if skating), water, and

Food: Snacks provided both days, please bring your own snacks if you have dietary restrictions.
